Job summary

Sentara Health Plan PG is seeking a Behavior Health Utilization Management Care Coordinator to manage complex Behavioral Health cases. The role requires both in-person and telephonic assessments for members with Serious Mental Illnesses, SED, and Substance Use Disorders.

Applicants must be in-state or willing to travel within VA and surrounding areas. Required: Bachelor's degree and valid licensure in VA or nearby state; 2 years behavioral health and 1 year case management experience.

Qualifications

• 2 years of behavioral health experience required.
• 1 year of case management experience required.
• Licensure preferred: LCSW/LPC/LMFT or RN (optional certifications accepted).

Responsibilities

• Conduct in-person and remote assessments for members with complex behavioral health needs.
• Coordinate utilization management across medical and behavioral health services.
• Collaborate with care teams to develop patient-centered plans and ensure timely access to services.
