Job summary

Collage Rehabilitation Partners is seeking a Behavior Health Tech for the Lakewood, CO residential programs. This full-time 2nd shift position entails direct care to individuals with brain injury and behavioral challenges, including documenting, assisting with ADLs, and facilitating activities.

The role requires a high school diploma or GED, being at least 18, a valid driver's license, ability to lift 50 lbs, and strong documentation skills. Comprehensive benefits are offered with this role.

Responsibilities

• Implements facility wide behavioral training as well as individual behavioral protocols, completing the necessary documentation.
• Models appropriate behavior provides positive reinforcement and corrective feedback on a daily basis.
• Assists residents in ADL’s (Activities of Daily Living includes bathing, transfers, toileting and physical assistance), assists and participates in physical, vocational and recreational activities.
• Transports residents to medical appointments, vocational, recreational, educational activities in the community.
• Completes all daily needed documentation (i.e. behavioral data, token exchange, shift summaries, house charts, checklists, medication administration records, and incident reports).
• Completes all housekeeping chores (i.e. laundry, vacuuming, dusting, etc.)

For more than 40 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries.

Hourly Rate: $20-22 per hour based upon experience and education

Schedule: 4PM-12AM
