Job summary

Northwestern Medicine is seeking a Case Manager RN UR/Bed Planner in Illinois to determine appropriate level of care and admission status, coordinating bed placement and patient flow. You will collaborate with physicians and nursing leadership to ensure smooth admissions and efficient utilization of resources.

Responsibilities include reviewing criteria, managing transfers in the EMR, and analyzing throughput and Medicare criteria while supporting risk management and quality teams.

Qualifications

• Registered Nurse license issued by the State of Illinois.
• BSN degree required; MSN preferred.
• Experience in admissions, utilization review, discharge planning, or case management.

Responsibilities

• Determine appropriate level of care and admission status using approved criteria and guidelines.
• Collaborate with physicians on hospital status orders and bed placement.
• Coordinate bed requests, throughput, and transfer Orders in the EMR.
• Monitor Medicare/diagnosis related criteria and avoidable days; report throughput issues.
• Assist with data collection and reporting for bed placement and throughput.
• Support risk management, infection control, and quality departments as needed.
