Qualifications

Responsibilities

• Under supervision from the clinical care team, provides care coordination for patients as they move into and through the care continuum.
• Responsible for insurance verifications, patient registration, and timely payor notification for maximum reimbursement.
• Facilitate benefit coverage for continuum of care needs and ensure flow of information to care providers.
• Maintain a high level of confidentiality in handling patient information.
