• Active Connecticut RN license in good standing.

• Valid driver's license and reliable transportation.

Responsibilities

• Complete Start of Care (SOC), Resumption of Care (ROC), and other skilled nursing visits.
• Perform comprehensive patient assessments in patients' homes.
• Accurately complete OASIS documentation.
• Develop and coordinate individualized care plans.
• Educate patients and caregivers regarding diagnoses, medications, and treatment plans.
• Collaborate with physicians and interdisciplinary team members to ensure quality patient outcomes.
• Maintain timely, accurate, and compliant clinical documentation.
