Job summary

Hartford HealthCare Medical Group seeks an RN to deliver skilled nursing care across clinical settings under provider direction. You will assess needs, plan care, and coordinate continuity of patient care with the care team.

This role requires a current CT RN license, willingness to travel between offices, and prior experience in a medical/surgical office. Knowledge of medical software is a plus; Hartford HealthCare offers career growth and comprehensive benefits.

Qualifications

• Current Connecticut RN License is required.
• Previous experience in Medical/Surgical Office setting preferred.
• Experience with medical software is preferred.

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention in prompt, precise, and professional manner.
• Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy.
• Travel between offices is required.
