Job summary

Hartford HealthCare Medical Group is seeking a registered nurse to provide skilled nursing care across multiple clinical settings under provider direction. The role involves collaboration with physicians, patient assessment, care planning, and ensuring continuity of care.

Ideal candidates have a Connecticut RN license, experience in a medical/surgical office, and familiarity with medical software. Bilingual Spanish skills are preferred. Travel to Cheshire for training and coverage is required.

Qualifications

• Current Connecticut RN License required.
• Previous experience in Medical/Surgical Office setting preferred.
• Experience with medical software.
• Bilingual in Spanish preferred.

Responsibilities

• Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings.
• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner.
• Travelling to Cheshire for training and coverage purposes.
