Qualifications

• Master’s degree in Psychology, Social Work, Counseling, or related field, or Doctorate-degreed Psychologist.
• Active, unrestricted license to practice in Arizona (LCSW/LMFT/LPC/LP).
• 2+ years of experience in a mental health environment.
• 1+ years of experience working with local/state resources in Arizona.
• Proficient in MS Office; reliable home workspace with high-speed internet.
• Permanent residency in Arizona.

Responsibilities

• Engage and manage behavioral health and substance use member cases.
• Complete member assessments; educate on resources and services.
• Coordinate with providers and treatment teams; address barriers to care.
• Participate in Interdisciplinary Team Rounds for high-risk members.
• Operate within Integrated Care Management Model to ensure needed services.
