Additional Information About the Role

  • LCSW required
  • Days 5– 8 hrs shifts Mon–Fri start times can be between (8 and 10 am)

Role Purpose

This position provides services to address the psychosocial needs of patients and families experiencing intimate partner violence. The role provides individual and group counseling, linkage to community resources, as well as expertise in cultural diversity and advocacy. It collaborates with the health care team and ensures timely, cost–effective discharge and transition planning.

Responsibilities

  • Assess and evaluates patient's physical, psychosocial, and emotional needs through initial contact and develops a safety plan to ensure the immediate discharge safety needs of the patient are met.
  • Establishes consistent, open, and trusting relationships with clients to understand their needs, resources, perspectives, and culture using discretion for confidentiality.
  • Works in partnership with clients to enhance their strategies and plans by identifying and analyzing options and resources that are relevant, available, and accessible to them.
  • Provides enhanced advocacy by assisting clients in navigating systems and services through community resources and vendors such as accompaniment to appointments as needed, developing life–skills, and achieving financial stability.
  • Develops and facilitates educational materials and programs to educate team members and the community.
