Qualifications

• High school diploma or GED; read, write and comprehend English to follow care plans.
• MAP (Medication Administration Program) certification; CPR/First Aid training offered within set timelines.
• Valid driver’s license with at least one year of driving experience.

Responsibilities

• Assist individuals with daily living activities (hygiene, feeding, dressing, grooming, meal prep).
• Transport individuals using wheelchair accessible vans to appointments and activities.
• Complete ISP to support personal goals and independence; maintain daily logs and behavioral data.
• Support program safety, confidentiality, and dignity; document services accurately.
