Qualifications

• High school diploma or GED equivalent.
• Pass a comprehensive criminal background check.
• Strong writing skills and attention to detail, especially for protocols and care instructions.
• Complete agency-required training including CPR, First Aid, CDS initial certification and orientation within 30 days.
• Maintain professional certifications and comply with training requirements.
• Ability to assist with personal care tasks and maintain client safety and dignity.
• Flexible hours as needed by house and client needs.
• Follow sequential written and verbal directions accurately.
• Utilize documentation software and report concerns promptly.

Responsibilities

• Monitor and assist with daily personal tasks including bathing, dressing, hygiene and toileting.
• Plan and implement client activities and community engagement per individual preferences.
• Maintain nutritional needs and prepare prescribed meals.
• Perform housekeeping tasks to meet licensing standards.
• Complete daily logs, notes, data tracking and incident reports on schedule.
• Report facility maintenance issues and coordinate with appropriate teams.
• Participate in assessments, PSP preparation, and training plan development.
• Attend mandatory staff meetings and shift handoffs.
• Overtime may be required with time-and-a-half for hours over 40 per week.
• Awake night duties including bed checks and nocturnal safety monitoring.
