Job summary
FOX Rehabilitation is expanding in Indianapolis, IN and seeking licensed Speech Language Pathologists to deliver one-on-one care in homes or Senior Living communities. You’ll develop care plans, deliver evidence-based treatment, and collaborate with a multidisciplinary team while maintaining a flexible schedule and the option for telehealth.

Qualifications
- Valid Speech Language Pathology license in the state of practice or eligibility to obtain.
- Degree from an accredited SLP program with CCC preferred or eligibility.
- Current CPR certification and EMR proficiency.
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
- Perform evaluations, develop plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions supporting wellness, longevity, mobility, and independence.
- Document effectively in FOX’s EMR system per regulatory standards.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote aging in place and safety.
