Job summary

Elara Caring is hiring a Registered Nurse Home Health to provide high-quality in-home nursing care across our patient population. You will deliver care in patients’ homes, coordinate with an interdisciplinary team, and support families while maintaining documentation and HIPAA compliance.

The role demands travel within the service area, lifting capabilities, and a commitment to compassionate, patient-focused care.

Qualifications

• Active RN license in the state
• 1+ year nursing experience
• Willingness to travel 50%
• Able to lift up to 50 pounds
• Valid driver’s license and insured vehicle
• Associates or Bachelor’s in Nursing preferred
• Home Health/Hospice experience preferred
• Experience with HomeCare HomeBase preferred

Responsibilities

• Align care with Elara Caring’s mission and leadership
• Maintain patient and staff privacy under HIPAA
• Assess home health patients and families to identify needs
• Assume caseload responsibility and plan, implement, and evaluate care
• Assess factors influencing patient health and incorporate into care plan
• Perform initial evaluation and re-evaluations of nursing needs
• Schedule and conduct follow-up visits as needed
• Communicate findings to Clinical Supervisor, physician, and team
• Revise plans of care in response to patient needs
