Qualifications

• Education: B.S. in Physical Therapy from an APTA-approved program.
• Licensure: Current State License or multi-state PTLC license where applicable.
• Experience: 1 year in acute, rehab, or home health preferred.
• Skills: therapy skills per standards; strong interpersonal skills; CPR certification; knowledge of durable medical equipment.

Responsibilities

• Develop and implement individualized physical therapy treatment plans.
• Evaluate and document patient progress; adjust treatments as needed.
• Coordinate care with other health professionals to ensure comprehensive treatment.
• Educate patients and families on plans, home exercises, and preventive care.
• Maintain accurate patient records per standards and regulatory requirements.
• Travel locally; work in patients' homes or healthcare facilities.
