Job summary

FOX Rehabilitation is seeking licensed Speech Language Pathologists to deliver one-on-one care in homes and Senior Living Communities in Enfield, CT. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes with telehealth options to extend access.

Responsibilities include evaluations, treatment planning, and collaboration with an interdisciplinary team to promote safety, function, and aging in place for older adults.

Qualifications

• Must hold a valid Speech Language Pathology license in the state(s) of practice or eligibility to obtain it.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC).

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in home, senior living communities, or other settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility and independence.
• Document effectively within FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
