Job summary
FOX Rehabilitation is growing in Collegeville, PA, and seeks licensed Speech Language Pathologists to deliver one-on-one care to older adults in homes and senior living communities. You will conduct evaluations, develop individualized care plans, and provide evidence-based therapy focused on mobility and independence.
You'll join an interdisciplinary team and may incorporate telehealth to extend access and flexibility, while enjoying autonomy in scheduling and meaningful patient outcomes through
Qualifications
- Valid Speech Language Pathology license in the state of practice
- Master's degree in Speech-Language Pathology
- ASHA CCC certification or eligibility
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings
- Perform evaluations, develop plans of care, and deliver evidence-based treatment
- Deliver therapy sessions to support wellness, mobility, and independence
- Document in FOX's EMR system per regulatory standards
- Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
- Educate patients, caregivers, and staff to promote safety and aging in place
- Provide telehealth/virtual care when appropriate to extend access
