Job summary

VitalCaring Group seeks a Home Health Speech-Language Pathologist to evaluate and treat patients with speech, language, cognitive, and swallowing disorders in their homes. You will develop personalized care plans and deliver evidence-based therapies, coordinating with nurses and other clinicians to optimize outcomes.

You will travel between patient homes, maintain thorough documentation in the EMR, and advocate for patient safety and independence while adapting interventions to real-world

Qualifications

• Must have a Master’s or Doctorate in Speech-Language Pathology.
• Current SLP license in the state of practice.
• 1 year clinical experience (home health preferred).
• CCC-SLP preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.
• Travel between patient homes; CPR certification and valid driver’s license required.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive, and swallowing function.
• Develop and implement individualized plans of care to improve communication, cognition, and safety.
• Provide therapeutic interventions using evidence-based techniques and tools.
• Assess the home environment and recommend adaptive equipment or communication aids.
• Educate patients and caregivers on exercises and safety techniques.
• Monitor progress, reassess as needed, and adjust treatment plans accordingly.
• Document all care accurately within the EMR.
• Collaborate with nurses, therapists, and interdisciplinary teams for coordinated care.
• Participate in case conferences, discharge planning, and ongoing care coordination.
