Job summary

VitalCaring Group in the Tampa area is seeking a dedicated Registered Nurse (RN) to lead home health care for patients in their homes. You will assess, plan, and deliver skilled nursing services, coordinating with physicians and the care team to drive positive outcomes.

This field-based role offers autonomy with a collaborative clinical team, opportunities to strengthen patient independence, manage care transitions, and document care in the EMR.

Qualifications

• Active RN license and CPR certification.
• Minimum 1 year RN experience in Home Health, Med-Surg, ICU, or Acute Care.
• Strong clinical judgment and independent thinking; excellent communication.

Responsibilities

• Conduct in-home assessments to evaluate clinical status and goals.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including meds, treatments, and interventions.
• Coordinate care across therapists, aides, and physicians.
• Educate patients and families for disease management.
• Document in EMR and participate in on-call rotations.
