Job summary

VitalCaring Group is seeking a PRN Weekend Home Health Registered Nurse to lead in-home care from admission to discharge, with strong autonomy. You will assess, plan, and manage care with physicians and a multi-disciplinary team, aiming to reduce hospitalizations and improve recovery at patients' homes.

The role emphasizes flexible weekend scheduling, independent decision-making, and high-quality patient education, with strong support from a collaborative clinical team to ensure optimal outcomes.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• 1+ year RN experience (Home Health preferred)
• Strong clinical judgment and independent thinking
• Excellent communication and patient-centered approach
• Caseload management and prioritization
• Proficiency with EMR systems
• Valid driver’s license and reliable transportation

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as case manager coordinating across therapists, aides, and physicians
• Monitor patient progress and adjust care based on changing conditions
• Educate patients and families for disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely documentation in the EMR
• Supervise Home Health Aides as needed
• Participate in on-call rotation as appropriate
