Fox Rehabilitation in Exeter, NH is seeking licensed Speech Language Pathologists to deliver one-on-one care to older adults in homes or Senior Living Communities under the Geriatric House Calls model.

As an SLP, you’ll evaluate, create plans of care, deliver evidence-based treatment, and collaborate with an interdisciplinary team. You’ll enjoy autonomy, flexible scheduling, and opportunities to use telehealth where appropriate.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Speech Language Pathology program.
• Current Certificate of Clinical Competence (ASHA CCC).
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsabilités

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in the EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
