Qualifications

• Active NP or PA license in applicable state.
• Board certification in family or internal medicine types preferred.
• Strong leadership and communication skills.
• Experience with senior care or chronic disease management is a plus.

Responsibilities

• Deliver high-quality primary care to residents across communities.
• Develop individualized treatment plans and manage acute and chronic conditions.
• Build trusting relationships with residents, families, and care teams.
• Document care in EMR accurately and timely.
• Collaborate with physicians and staff to coordinate care.
• Support quality improvement initiatives and avoidable admissions.
