Job summary

Sobe Innovative Rehab is seeking a licensed PTA to deliver patient-centered care to older adults in Assisted Living Facilities and occasional home visits. The role emphasizes clinical autonomy, collaborative support, and maintaining a full caseload within a 9:00 AM–5:30 PM schedule.

The ideal candidate has 3+ years of PTA experience, strong documentation skills, and a compassionate, patient-focused approach to helping older adults stay active and independent.

Qualifications

• Licensed PTA with 3+ years of experience.
• Ability to work with older adults in ALFs and homes.
• Strong documentation within 48 hours.
• Proactive, patient-focused and reliable.

Responsibilities

• Manage a full caseload and provide individualized treatments.
• Primarily treat patients in Assisted Living Facilities (ALFs), with occasional home/community visits.
• Manage your patient schedule within the 9:00 AM–5:30 PM workday.
• Communicate with patients, families, caregivers, and clinical staff.
• Exercise clinical autonomy while collaborating with experienced therapists.
• Complete documentation within 48 hours.
• Help patients improve strength, mobility, balance, function, and quality of life.
