Job summary

Magnolia Creek Nursing and Rehabilitation is seeking a qualified Physical Therapist to provide comprehensive PT services, evaluation, treatment planning, and discharge planning within our in-house program.

You will collaborate with nursing and rehab teams, supervise PTAs and Rehab Techs, and ensure compliance with regulations while delivering high-quality patient care and maintaining professional standards.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state Physical Therapist license.
• Proficient with Microsoft Office applications and basic internet navigation.

Responsibilities

• Provide physical therapy services, including evaluation and treatment as referred by an approved source.
• Develop initial and ongoing treatment plans of care and discharge planning.
• Supervise PTAs and Rehab Techs in accordance with state practice acts.
• Ensure compliance with state and federal regulations governing physical therapy practice.
• Participate in utilization review, care conferences, and interdisciplinary team activities.
• Maintain professional conduct and uphold patient rights and confidentiality.
• Continuing education and professional development to enhance clinical expertise.
• Perform other duties and responsibilities as assigned.
