Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for Queenstown, Maryland to deliver in-home and senior living care under the Geriatric House Calls model. You’ll conduct evaluations, develop plans of care, and deliver evidence-based therapy to older adults in their homes or communities.

The role offers autonomy, flexibility, and opportunities to grow with telehealth options and a collaborative, interdisciplinary team focused on meaningful outcomes for patients and families.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility, and independence; document in the EMR per standards.
• Collaborate with an interdisciplinary team and educate patients, caregivers, and staff.
