Job summary

21st Century Home Health Services is seeking a dedicated Physical Therapist Assistant to provide in-home care across the Bay Area, including Marin and Sonoma counties. The role requires active California PTA licensure, CPR, a valid California driver’s license, and comfort working independently in patients’ homes.

You will assess patients, develop personalized treatment plans, and collaborate with an interdisciplinary team to optimize mobility and quality of life.

Qualifications

• California PTA license is required.
• Home health experience is highly preferred.
• Current CPR certification is required.
• Current California driver's license is required.
• Computer proficiency with charting systems.

Responsibilities

• Examine each patient and develop a plan using treatment techniques to promote mobility, reduce pain, restore function, and prevent disability.
• Work with patients to prevent loss of mobility by developing fitness- and wellness-oriented programs.
