Job summary

Legacy Care in Charlottesville, Virginia is seeking a dedicated Nurse Practitioner to provide high-quality medical care in post-acute settings on a PRN basis. You will work with facility staff and a multidisciplinary team to support continuity of care and positive patient outcomes.

This role offers autonomy within a supportive clinical and administrative team, with PRN scheduling, professional development support, and competitive compensation.

Qualifications

NP license and DEA registration required in Virginia.
Experience in post-acute care or geriatrics preferred.
Strong clinical judgment and communication skills.

Responsibilities

Assess patients, develop and review care plans, and monitor progress in post-acute settings.
Provide patient-centered medical care in line with standards and requirements.
Collaborate with nursing, physicians, facility staff and the care team.
Communicate with patients, families and care teams on treatment plans.
Document care promptly using EMR systems.
Support care coordination and transitions between settings as needed.
