Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current, unrestricted LPN license in the state of Kansas.
• Home health or hospice experience preferred (training provided).
• Knowledge of basic nursing principles and patient care practices.
• Ability to pass a criminal background check.
• Current CPR certification with in-person skills verification required.

Responsabilités

• Provide skilled nursing under the direct supervision of a Registered Nurse.
• Assist the Registered Nurse in implementing the plan of care.
• Assist with patient rehabilitation and ongoing evaluation of the care plan.
• Observe, record, and report patient symptoms and changes in condition.
• Foster teamwork and participate in staff conferences.
• Prepare clinical and progress notes (excluding admissions).
