Job summary

One Home Health is seeking a motivated Physical Therapist Assistant (PTA) to support our home health patients on a part-time basis in the Chicago area. You will work with an interdisciplinary team delivering one-on-one therapy in patients' homes.

This role offers autonomy, flexible scheduling, and meaningful patient care, with opportunities to grow clinical skills and contribute to quality improvement initiatives.

Qualifications

• Current, unrestricted Physical Therapist Assistant license in Illinois.
• Graduate of an accredited PTA program recognized by the APTA.
• Minimum of six months PTA experience preferred.

Responsibilities

• Provide prescribed PTA treatments and patient education under the supervision of the Physical Therapist and physician-approved plan of care.
• Monitor patient progress and communicate changes to the team.
• Maintain timely clinical records meeting agency, state, and federal documentation requirements.
• Collaborate with nurses, therapists, physicians, and other healthcare professionals to promote positive patient outcomes.
• Assist with care coordination and participate in interdisciplinary team meetings when requested.
• Follow infection control practices, safety procedures, and equipment usage guidelines during home visits.
• Protect patient confidentiality and comply with HIPAA and privacy standards.
• Adhere to agency protocols, regulatory requirements, and professional practice standards.
• Participate in educational programs, staff meetings, and ongoing quality improvement initiatives.
• Carry out additional duties related to patient care and agency operations as assigned.
