Job summary

Advanced Home Health Care Ltd. is seeking a part-time Occupational Therapist to deliver in-home therapy across Southeast Iowa, covering Des Moines, Lee, Henry, Louisa, and Van Buren Counties. You will join an integrated, coordinated care team with autonomy over your day and a focus on high-quality patient outcomes.

You'll work in patients' homes, provide direct OT services, and collaborate with an interdisciplinary team while enjoying flexibility in scheduling and mileage reimbursement.

Qualifications

• Valid, active Occupational Therapist license.
• Relevant clinical experience.
• Strong communication and organizational skills.
• Collaborative, patient-centered approach.
• Commitment to providing high-quality care and improving patient outcomes.
• Preferred: previous home health experience.

Responsibilities

• Provide direct occupational therapy services to patients in their homes.
• Monitor patient progress and outcomes.
• Adapt recovery plans to each patient's individual needs and goals.
• Help patients improve mobility, independence, and overall well-being.
• Collaborate with an interdisciplinary healthcare team to provide coordinated, patient-centered care.
• Communicate effectively with patients, families, and other members of the care team.
• Travel throughout our Southeast Iowa service area to provide patient care.
