Masters In Home Care in Rocky Hill, CT seeks a dedicated Occupational Therapist to join our full-time team serving patients in homes and facilities.

You will implement and evaluate plans of care, coordinate with physicians, supervise aides, and use our clinical point-of-care system on a tablet.

Formación

• Current and active CT Occupational Therapist License in good standing.
• Graduation from an Occupational Therapy educational program.
• Experience with electronic documentation/point of care systems a plus.

Responsabilidades

• Provides OT services in the patient’s home setting following a plan of care established with the physician.
• Develops and evaluates the plan of care and makes revisions as needed.
• Coordinates with other health professionals to ensure continuity of care and identifies needs for additional services.
• Instructs and assists patients and caregivers in ADLs and compensatory strategies.
• Supervises aides and OT Assistants, if assigned.
• Supports real-time access to patient data via tablet-based systems.
