Job summary

UltaCare Medical is seeking a Medical Nurse Practitioner or Physician Assistant to deliver primary and acute care in post-acute settings. You will lead care, document accurately, and support residents with geriatrics-focused services.

The role offers a flexible schedule with no nights or weekends, company-paid health/dental/vision insurance, a 401(k), malpractice coverage, CME allowance, and ongoing education support through orientation and preceptorship.

Qualifications

• Unrestricted professional license.
• Ability to practice independently and complete accurate, timely documentation.
• Experience in geriatrics, preferred.
• Experience in post-acute care, preferred.

Responsibilities

• Medical management across primary and acute care, monitoring chronic conditions.
• Quality improvement to reduce hospitalizations and ED visits and improve function.
• Collaborate with nurses; support and guide practitioners with medical expertise.
• Ensure regulatory compliance.
