Qualifications

• Active and unrestricted LMHC license
• Strong written and verbal communication skills
• Ability to work independently within a collaborative team
• Commitment to timely documentation and high-quality patient care
• Willingness to provide on-site services in long-term care facilities

Responsibilities

• Deliver individual, family, and group therapy to long-term care residents
• Complete clinical assessments, psychosocial evaluations, and treatment plans
• Support patients with depression, anxiety, grief, trauma and other mental health concerns
• Provide brief, goal-oriented interventions for older adults with complex conditions
• Collaborate with nursing, social services, rehabilitation, and medical teams to coordinate care
