Job summary

FOX Rehabilitation is seeking a licensed Speech Language Pathologist to provide in-home and senior living care in the Poughkeepsie, NY area. Build one-on-one patient relationships, conduct evaluations, and create individualized plans of care for older adults, including telehealth when appropriate.

You’ll work autonomously in private residences or senior living communities, collaborating with PTs, OTs, SLPs, and Fitness Specialists to improve mobility, independence, and quality of life.

Qualifications

• Active Speech Language Pathology license in NY or eligibility to obtain
• Current CPR certification
• Master's degree in Speech-Language Pathology
• ASHA CCC certification

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to enhance wellness, longevity, mobility, and independence
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
