Job summary

FOX Rehabilitation in Springfield, IL seeks a licensed Speech Language Pathologist to provide one-on-one care in patients' homes and senior living communities. The role blends in-home visits with optional telehealth, offering flexible scheduling and meaningful patient outcomes.

Qualifications

• Valid Speech Language Pathology license in the state(s) of practice.
• Degree from an accredited Speech Language Pathology program.
• Current CPR certification.
• Current Certificate of Clinical Competence (ASHA CCC) or eligibility to obtain CCC.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively in FOX's EMR system according to standards.
• Collaborate with interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
