FOX Rehabilitation in Arlington Heights, IL is seeking a licensed Speech Language Pathologist to deliver one-on-one care to older adults in homes or senior living communities, using our Geriatric House Calls model. Qualifications

• Degree from an accredited Speech Language Pathology program.
• Current Certificate of Clinical Competence (ASHA CCC) is required.
• CPR certification is current and valid.

Responsibilities

• Provide one-on-one Speech Language Pathology services to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.

Education
