Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in private homes or Senior Living Communities across Lecanto, FL. You’ll develop personalized plans and provide evidence-based therapy to improve mobility, independence, and quality of life through our Geriatric House Calls model.

Autonomy and flexibility are central—managing your own schedule while maintaining strong patient relationships.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access and flexibility.
