Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care in Centereach, NY, including private homes and senior living communities. You’ll build strong patient relationships and tailor plans of care to improve mobility and independence.

The role offers autonomy, flexible scheduling, and potential telehealth integration to extend access while maintaining high-quality, patient-centered outcomes for older adults.

Qualifications

• Must hold a valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer/EMR proficiency and CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Develop evaluations and individualized plans of care.
• Deliver therapy to improve mobility, independence, and function.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness staff.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
