Job summary

Interim HealthCare Home Health and Hospice is seeking a Hospice RN Case Manager in Portage, MI. You will provide skilled nursing care in patients' homes with autonomy to manage your day and support families through difficult times.

The role requires an active Michigan RN license and CPR certification, with at least 1 year of clinical experience. The position offers a salary range of $75,000 to $100,000 annually plus weekly bonus opportunities and comprehensive benefits.

Qualifications

• Registered Nurse licensed in Michigan and CPR certified.
• At least 1 year of clinical nursing experience, preferably Hospice.
• Ability to work independently in patient homes and coordinate with a team.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Oversee CNAs and collaborate with physicians on patient care.
• Assess patients, document changes, and report to the physician.
• Educate patients on plan of care, medications, diet and exercise.
• Coach family members on the care plan and home safety.
• Coordinate discharge from services when goals are met.
