Job summary

VitalCaring Group in Texas is seeking a Field-Based Home Health RN to lead patient care at home, coordinating plans of care, and driving outcomes from admission to discharge.

You’ll perform in-home assessments, develop individualized care plans with physicians, deliver skilled nursing care, and educate families while maintaining timely EMR documentation.

This role offers strong autonomy, interdisciplinary support, flexible scheduling, and meaningful patient connections across a territory.

Qualifications

• Active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience (Home Health/Med-Surg/ICU/Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
