Job summary

VitalCaring Group is seeking a Field-Based Home Health RN to lead patient care in home settings across an assigned territory. You will assess, plan, and provide skilled nursing services, coordinating with physicians and a multidisciplinary team to achieve optimal outcomes.

You will maintain high-quality documentation, participate in on-call rotations, and support patient independence through education and meticulous care planning. This role emphasizes autonomy with collaborative clinical support.

Qualifications

• Must have active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Proficiency with EMR systems.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
