Job summary

VitalCaring Group is seeking a Field-Based Home Health RN to lead care for patients in their homes with clinical autonomy and a focus on outcomes. The role emphasizes comprehensive assessment, care planning, and collaboration with physicians and interdisciplinary teams.

You will assess, plan, and deliver skilled nursing care, educate patients and families, and coordinate transitions from admission to discharge in a dynamic, home-based setting.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health preferred).
• Strong clinical judgment and patient-centered approach.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate
