Job summary

Banner Health is seeking a Home Health Registered Nurse to deliver autonomous, in-home patient care across West Valley territories. You will assess, plan, implement, and evaluate nursing care within the patient’s home setting, coordinating with the care team and caregivers.

The role emphasizes individualized patient relationships, documentation in Home Base EMR, mileage reimbursement, and flexible scheduling to support patient outcomes and staff satisfaction.

Qualifications

• RN licensed to practice in the state of Arizona.
• Current BLS certification required.
• Experience in Home Health or acute care is preferred.

Responsibilities

• Assess patients’ physical, psychological, social, and discharge needs.
• Plan and coordinate individualized nursing care.
• Provide direct patient care and supervise licensed staff as needed.
• Document care and communicate with physicians and family.
• Participate in quality improvement and ongoing training.
