Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager in Oklahoma City. You will play a hands-on role in developing and adjusting plans of care with LPNs, therapists, and physicians, delivering patient-centered care and strong clinical judgment.

You’ll enjoy autonomy, flexible scheduling, and a team that values the patient experience. Active Oklahoma RN license and CPR are required, with home health experience welcomed.

Qualifications

• Active RN license in Oklahoma.
• CPR certification is required.
• Home health experience is desirable but not mandatory.
• Strong assessment and critical thinking skills are essential.

Responsibilities

• Develop and adjust the plan of care with the care team.
• Complete visits, OASIS assessments, and documentation with a focus on the patient.
• Coordinate with LPNs, therapists, and physicians to ensure continuity of care.
