Interim HealthCare of Oklahoma City is seeking a Home Health RN Case Manager in Oklahoma City to shape the plan of care for patients, collaborating with LPNs, therapists, and physicians. You’ll provide hands-on nursing in patients’ homes and build meaningful patient relationships.

Qualifications

• Active RN license in Oklahoma.
• Home health experience is ideal, but we're also looking for experienced nurses from any clinical background who are ready for a change and bring strong assessment and critical thinking skills.
• CPR certification.
• A nurse who wants autonomy, real patient relationships, and a genuine say in the plan of care.

Responsibilities

• Play a hands-on role in developing and adjusting the plan of care for patients with the care team.
• Complete visits, OASIS assessments, and documentation with a focus on the full patient picture.
• Coordinate with the care team so nothing falls through the cracks.
• Educate patients and families on diagnosis, medication, and home safety.
• Advocate for patients and help drive real outcomes.
