The position offers flexible hours (32 or 40 per week) with limited on-call requirements and travel within the Greater Augusta area. Candidates must hold an active Maine RN license and have at least 1 year of acute or med/surg nursing experience, with EMR proficiency preferred.

Qualifications

• Active Maine RN license or ability to obtain promptly.
• At least 1 year of acute or med/surg nursing experience.
• Home health or case management experience preferred.
• Valid driver’s license with reliable transportation.

Responsibilities

• Consult with nursing staff and multidisciplinary team to evaluate patient status and care plans.
• Provide direct nursing care and education, and coordinate services.
• Develop and evaluate care plans to optimize patient outcomes.
• Monitor patient condition and document in EMR.
