Job summary

MaineGeneral Health is recruiting an RN Home Health in Waterville, Maine to provide in-home patient care and build meaningful patient relationships at the point of care.

You will work with the care team to assess status, manage treatments, and develop care plans aimed at improving outcomes while ensuring patient safety and comfort.

Qualifications

• Active Maine RN license
• At least 1 year of acute care, med/surg, or equivalent nursing experience (home care, ED, ICU, or CCU experience preferred)
• Valid driver’s license and acceptable driving record

Responsibilities

• Collaborate with nursing staff and care team to assess patient status and care decisions such as admission, length of stay, transfer and discharge.
• Deliver direct treatment, education, and health condition management, advising providers on additional services and community referrals.
• Develop, implement, and assess patient care plans based on assessments to improve outcomes; monitor, record, and communicate patient condition as needed.
