Job summary

Banner Home Health in Casa Grande, Arizona is seeking a dedicated Physical Therapist to evaluate, plan and supervise in-home therapy under physician referrals and established standards.

You will enjoy an autonomous but supportive work environment within Banner's home care network, serving East Valley PODs with manageable travel distances. A pay-per-visit or hourly option, mileage reimbursement and documentation tools are provided.

Qualifications

• Graduated from an APTA-accredited school of Physical Therapy.
• Current Physical Therapy license in the practicing state.
• BLS certification required.
• Ability to articulate clinical competency and education during interview process.
• Proficiency with computer software used for documentation.

Responsibilities

• Evaluates patients' functional abilities; establishes and prioritizes goals; develops an individualized plan of care.
• Implements plan of care; documents progress and communicates with care team.
• Supervises and delegates tasks; participates in training and orientation of personnel.
• Ensures departmental processes, productivity and quality outcomes.
