Job summary

Banner Health is seeking a Part-time Home Health Physical Therapist in Mesa, AZ. You will evaluate patients, plan and implement therapy, and document progress in Home Care Home Base. The role supports autonomy with occasional travel across east valley POD 6 service areas.

Responsibilities include patient and family education, collaboration with physicians and nursing teams, and adherence to regulatory standards. Training is provided, with a typical daytime schedule and per-visit compensation.

Qualifications

• Graduation from an APTA accredited school of Physical Therapy or equivalent.
• Current Physical Therapy license in the state of practice.
• BLS certification required.

Responsibilities

• Evaluate patients’ functional abilities and establish a plan of care with measurable goals.
• Implement and adjust individualized treatment plans.
• Document care in accordance with professional guidelines and timelines.
• Communicate results and progress to the care team and patients.
• Supervise and delegate tasks to support staff as appropriate.
• Participate in department meetings and process improvement initiatives.
