Job summary

Trinity Health Of New England At Home is seeking a Home Care Physical Therapist to deliver patient‑centered therapy in patients’ homes across the Stafford/Manchester/Mansfield region. You will develop individualized care plans, collaborate with physicians, nurses, social workers and home health aides, and work autonomously with high motivation and organization.

The role is full time with a $10,000 sign‑on bonus paid in installments.

Qualifications

• Degree incl. internship from an APTA‑approved physical therapy program.
• Current Connecticut Physical Therapist license.
• Minimum 1 year as a PT or completion of 6–8 week preceptorship.
• Familiarity with home healthcare and regulatory requirements preferred.

Responsibilities

• Plan and prioritize visits based on client needs and physician orders.
• Assess clients at admission and key intervals and document accurately.
• Develop and update care plans with measurable goals with physician and client.
• Provide therapy using approved techniques, exercises, and modalities.
• Coordinate care with other disciplines as needed.
• Educate clients and caregivers on therapy programs and home exercises.
• Monitor progress and report changes to the physician.
• Complete documentation promptly at point of care.
