Zusammenfassung

Trinity Health Of New England At Home is seeking a Physical Therapist for home care, Monday through Friday, 8 am to 4:30 pm, with rotating weekends. The role offers a $10,000 sign-on bonus paid in installments and requires CT PT licensure, an accredited PT degree, and experience in home healthcare.

You will work with physicians, nursing staff, social workers and aides to deliver autonomous, patient-centered therapy.

Qualifikationen

• Degree including internship from an APTA-approved PT program.
• CT Physical Therapist license required.
• Minimum 1 year experience or completion of 6–8 week preceptorship.
• Valid driver’s license and reliable transportation.

Aufgaben

• Plan and prioritize visits based on client needs and physician orders.
• Assess clients at admission and key intervals and document accurately.
• Develop and update care plans with measurable goals in collaboration with physician and client.
• Provide therapy using approved techniques, exercises, and modalities.
• Coordinate care and referrals with other disciplines as needed.
• Educate clients and caregivers on therapy programs and home exercises.
• Monitor progress and report changes promptly to the physician.
• Ensure compliance with laws, regulations, and agency policies.
• Complete documentation at point of care in a timely manner.
